Changes to HbA1c analysis to capillary electrophoresis.

LabPlus will shortly be changing method for HbA1c analysis from Primus affinity chromatography to capillary electrophoresis (Capillarys, Sebia).   The new method gives greater analytical precision and peak separation and can also identify the potential presence of haemoglobin variants.  

LabPlus HbA1c results have been within the Royal Australasian College of Pathologists (RCPA) Quality Assurance Programme allowable limits of performance, which specifies results within 4 mmol/mol at a level of 45 mmol/mol or below, and 8% above that (1).  

The new method produces results which are on average 2‐4 mmol/mol lower than previously. Statistical variation means there will be some patients where the difference between results may be greater than 2‐4 mmol/mol and others where no change is seen.

In addition to this analytical variation, HbA1c also shows individual biological variation of about 2‐4%, even in the absence of any change in glycaemic control (2).  This should also be considered when comparing results.
